[Attention]Request to prevent the spread of COVID-19 infection
You are kindly requested to observe rules or take necessary measures to prevent the spread of COVID-19 infection as below:
- 1. Those who meet following conditions shall not attend the forum:
A) Experiencing symptoms of physical deconditioning, such as fever of 37.5 degree Celsius or, extreme tiredness, cough, sore throat, taste disorder
B) Having experienced of symptoms of physical deconditioning such as fever of 37.5 degree or more, extreme tiredness, cough, sore throat, taste/smell disorder within 14 days prior to the date of the participation
C) Having close contact with those who tested positive for COVID-19 within 14 days prior to the date of the participation
D) Family living together or somebody who is very close, with whom you contacted become suspected of being infected (e.g. PCR tested positive) within 14 days prior to the date of the participation
E) Having close contact with those visiting countries or regions to be covered under restrictions for entry into Japan or requirement for observation after the entry, or those living in these countries or regions within 14 days prior to the date of the participation
In case you find out that you meet conditions stipulated as above, please cancel your attendance through My Page of the web
- 2. We would like to ask your kind cooperation on the day of the forum as below:
A) Observe coughing etiquette, wear a mask (non-woven or cloth), Use hand sanitizer
B) Cooperate with taking temperature and submittal of necessary documents at the reception and staggered entering/exit
C) Social distancing (at least 1m) while standing in line or staying away from assigned seat
D) Take a seat assigned to you and refrain from speaking loudly
E) Refrain from eating and drinking inside the venue except for drinking for health reasons such as hydration
F) Come directly from your home and go straight home as much as you can
G) Cooperate with downloading COVID-19 COCOA (Contact- Confirming Application)
H) Cooperate with investigation by Public Health Center such as provision of action history in case being infected by COVID-19 within 14 days after the forum. Provide personal information to official organization such as Public Health Center, if necessary.
I) Cooperate with any other measures or instructions which NDF decided to prevent the spread of COVID-19.

③ Fill in and Sign a Behavior/Symptoms Checklist

Please bring or show on your phone one of the followings.
・The e-mail included applicant details you receive just after pre-registration
・Forum’s My Page, “Issue Participation Certificate”
If you apply by FAX, please bring the Participation Certificate which will be sent to you by mail separately.
- 2. COVID-19 Documents
①Proof of COVID-19 Vaccination Status (original, copy, and photo are acceptable)
Ex) Certificate of Vaccination for COVID-19 (temporary or official), Record of Vaccination for COVID-19
You need to complete 2nd dose of vaccination at least 14 days prior to participation
②Negative Test Result：PCR Test or Quantitative Antigen Test (original, copy, and photo are acceptable)
You are required to take a test in a medical or test institution, and a certificate is necessary to include following items.
‐Examinee Name ‐Negative Test Result
‐Specimen Collection Date (PCR Test Date) ‐Certificate Issuer
The test need to be conducted with the specimen taken after 3 days prior to participation.
Examinees need to cover 100% of test expenses
